Sales Tax Exemption

General Form - This general form may be provided to vendors to avoid being charged sales tax. 

Some stores require accounts to make tax-free purchases. We have the following set up:

• Walmart - Customer ID #790434 - (ID card to print)
• Walmart.com - Login information for tax-free online shopping
• Target.com - Login information for tax-free online shopping
• Office Depot - Tax-exempt account #84141134
• Home Depot - Tax ID #1509295373 - (ID card to print)
• Lowe's - Tax-exempt account #178000153 - (tax exempt form to print) 

W-9

 W-9- Some companies request this form before making a donation or contribution to the PTA.
